We understand the importance of meeting Ministry standards, documenting progress, and ensuring accountability. Our process includes:
Timely communication about resident progress, incidents, and successes
Attendance at case conferences and professional meetings
Access to updated care plans, behavioral strategies, and incident reports
Collaboration on funding requirements and service coordination
Families or case managers can start the process by completing our secure referral form online or contacting our intake coordinator.
We’ll meet to learn more about the individual’s needs and arrange a home visit.
We collaborate with the individual, family, and case manager to create a tailored support plan.
We provide a gradual, supportive transition period to help the resident feel at home.
